Innovative graduate public health programs and medical school departments with a population health focus are encouraged to serve as models to aid the development and expansion of meaningful public health practice community-based learning opportunities and interprofessional education initiatives, both with a focus on improved health outcomes.

By applying to become an Academic Innovation Model (AIM), public health and health professions academic departments can gain credit for innovative work at your institution and be recognized as an APTR-CDC AIM. |
Instructions
To become an academic innovation model, the form will require a background description of the institution or program, a series of descriptions of innovative activities performed, and relevant resources for other academic institutions to access and model.
